Social prescribing is a new way of approaching personalised care, giving people the chance to share in the decision-making around their care and support in planning, self-management, help with their personal heath budgets and broader choice within
the NHS.
Manor Gardens' Social Prescribing Link Workers help individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health access and outcomes. They will provide support around the wider determinants of their health,such as debt, poor housing and physical inactivity, as well as other lifestyle issues and low-level mental health concerns by increasing people’s active involvement with their local communities.
Based alongside GP surgeries our staff link up with services across the borough as well as those based at Manor Gardens.
