Referrals

All partner organisations can use this form to refer a client to Manor Gardens Welfare Trust. Each referral will be assessed by a member of our staff, who will consider the most appropriate support available to the client, either through our own services or through those of partners organisations in the voluntary and statutory sectors. We are also able to offer a menu of wellbeing and practical support opportunities.

We are able to provide support to people who have complex needs and who would benefit from an holistic approach to their care. We work with people who do not meet the threshold for statutory services such as Adult or Children's Social Care, but are still in need of support to improve their wellbeing.

Our support includes social activities in person and online, welfare checks, form filling, and therapeutic groups. We provide advocacy support around housing, benefits, employment, health, immigration and related issues.

We undertake to work on each referral within three working days. Our working hours are Monday to Friday, 9am to 5pm. We are not a crisis service and clients in need of emergency help should not be referred to us.

By filling out this form you confirm that you have the client's consent to share their data with us and our VCS partners. We are unable to receive referrals without the client's consent.
